You will need to come to the police department, 1009 Leonard Ave High Point NC, with a picture ID.  If you don't have a picture ID you will need to have 2 of the following:
   1.  Certified birth certificate issued by a government agency in the U.S., Puerto Rico, a U.S. territory,
        or Canada or U.S. Report of Consular Birth Abroad.
   2.  Original Social Security Card (not the metal or plastic card or laminated card or other
        reproductions)
   3.  Unexpired U.S. Military Identification, including DD-2, DD-214, Common Access Card and
        U.S. Military Dependents Card.
   4.  Valid, unexpired Passport from any nation.
   5.  Release orders from the Magistrate, showing a charge of DWI and civil revocation or their driver's
        license.
